The item must be WORN - she only reads your ten equipment slots - and it must have at least one card in it, otherwise she refuses. Failure chance is 10%: on a failure the cards and the item both survive (failedremovecards ...,3), but your Star Crumb, Yellow Gemstone and the whole Zeny fee are gone anyway. The three destructive outcomes she threatens you with (item destroyed, cards destroyed, or both) are commented out in the script and can never happen.
